Any information the user enters into the form will be sent directly to the recipient(s) according to plugin settings. When enabled in the plugin settings, details about each sent message will be stored in the WordPress database. SimpleForm – Form Maker to Chat Apps with Quiz, Poll and Multi Lead Notifications
SimpleForm is a drag-and-drop form builder that goes further than collecting submissions. Every form you create can send leads directly to WhatsApp, Telegram, Slack, or Google Sheets the moment someone hits submit. No Zapier. No third-party bridge. It just works. Most form plugins store data in your WordPress database and leave you to figure out what to do with it. SimpleForm routes leads where your team actually works. If you run support on Telegram, you get a message. If your sales team lives in Slack, they get notified. If you track everything in Google Sheets, the row appears automatically. Beyond forms, SimpleForm includes a full quiz builder with scoring and time limits, a poll creator with real-time vote tracking, custom login and registration forms, multi-step form layouts, conditional logic, and a lead management dashboard where you can filter, search, and export every submission. You can build anything from a short contact form to a multi-page application form with conditional logic between steps. Fields support text, email, phone, file upload, dropdowns, checkboxes, radio buttons, date pickers, signatures, and more. Conditional logic lets you show or hide fields based on what a user selects, so long forms only show what’s relevant to each person. Multi-step forms include progress bars with several style options. You can set your own button labels for each step. Forms can also appear as floating widgets anchored to the bottom-left or bottom-right corner of the page, which works well for lead capture without interrupting content. Seven built-in visual themes are included — Modern, Minimalist, Neomorphic, Glassmorphism, and others. You can apply a theme per-shortcode, so different forms on the same site can look different. A custom theme builder is also available if none of the presets fit. WhatsApp and Telegram Lead Notifications When someone submits a form, SimpleForm can redirect them to WhatsApp with the form data pre-filled in the message. The contact arrives in your WhatsApp with the lead’s information already typed out. This works on both mobile and desktop. For Telegram, SimpleForm sends the submission directly to a Telegram channel, group, or individual user via a bot. You configure the bot token and chat ID once, and every form submission after that appears as a Telegram message. No additional plugins are needed. This is the part most form builders don’t do natively. Typically you need a separate automation tool or a paid addon to route form data to a messaging app. In SimpleForm, both WhatsApp and Telegram routing are included free. Slack and Google Sheets Slack notifications arrive in whatever channel you specify, formatted with the form field labels and values. You can set different Slack channels for different forms. Google Sheets sync connects via OAuth. After authorizing your Google account, you pick a spreadsheet and map each form field to a column. Submissions appear as new rows in real time. Quiz Builder The quiz builder handles scored assessments, personality quizzes, timed exams, and knowledge tests. Each question can include an image, and answer options can also use images. You set which answers are correct, assign point values, and configure pass/fail thresholds. Additional options include question randomization, answer randomization, and a configurable time limit. After completing the quiz, users see a results summary. Detailed quiz analytics are available in the dashboard. Poll Creator Polls support single-choice and multiple-choice formats. You can require login before voting, set a closing date, and control whether results are visible before or after a user votes. Revote control lets you decide if someone can change their answer. Image-based answer options are supported. A dedicated analytics view shows vote counts and percentages over time. Embed a poll with: [simple_poll id="1"] Lead Management Every form, quiz, and poll submission is stored in a lead dashboard with date filtering (today, yesterday, last week, last month, custom range), search, and sorting. You can export all data as CSV, JSON, or PDF, or print directly from the dashboard. Individual entries can be deleted or bulk-deleted. A notification indicator in the admin menu shows when new submissions arrive so you do not have to refresh the leads page manually. Login and Registration Forms You can replace the default WordPress login and registration pages with custom forms built in SimpleForm. These support additional custom fields, redirect rules after login or registration, and email verification. WooCommerce login and registration forms can also be replaced. The comment form can be customized separately. All authentication options are under Settings > Auth Settings. Integrations WhatsApp, Telegram, Slack, Form Builder – Google Spreadsheet Integrations, Mailchimp, FluentCRM, Trello, Email notifications, and Cloudflare Turnstile for spam protection. All integrations are configured from a single Integrations settings screen in the WordPress admin. Form Analytics Each form has its own analytics view showing total views, total submissions, submission rate, error logs, redirect paths, and time-based charts. You can compare forms against each other to see which layouts or placements convert better. Competitor Analysis Search the WordPress plugin directory for a form builder and a short list of names comes up in nearly every result: Formidable Forms, WPForms, Fluent Forms, Forminator, and Ninja Forms. It is worth being clear about where SimpleForm actually sits among them, and where a narrower plugin like OneClick Chat to Order fits a different job entirely. WPForms AI Form Builder for WordPress is the most installed name in the category, and its free Lite version covers contact forms and native Stripe payments well. Native WhatsApp notifications are not part of that free tier: sending a WhatsApp message on submission requires the Twilio addon, which unlocks on the Plus plan and up, and Telegram routing needs a separate Zapier connection. SimpleForm’s Slack redirect and Telegram bot notifications work in the free version with just a bot token and chat ID, no paid addon and no Zapier account required. Fluent Forms Customizable Contact Forms, Survey, Quiz, & Conversational Form Builder ships a genuinely capable free tier, with Mailchimp and FluentCRM built in. Slack and Google Sheets sync, though, sit behind Fluent Forms Pro, and Telegram notifications are a Pro-only integration as well. SimpleForm includes Slack, and Telegram in the free version, so a support or sales team can wire up all four channels without a license key. Formidable Forms WordPress Form Builder for Contact Forms, Calculators, Quizzes & More is closer to an application builder than a lead-routing tool. Formidable Views turns submissions into directories, listings, and searchable databases, which SimpleForm does not attempt. But getting a Formidable entry into Slack or WhatsApp means installing Uncanny Automator, or a third-party bridge like Zapier or Bit Integrations, on top of the form plugin itself. SimpleForm sends to WhatsApp, Telegram, Slack, and Google Sheets directly, with no separate automation plugin involved. Ninja Forms The Contact Form Builder That Grows With You keeps its core builder free, but conditional logic, multi-step forms, and every messaging integration (Slack, WhatsApp, Telegram) are paid add-ons or routed through Zapier. Forminator Forms – Contact Form, Payment Form & Custom Form Builder is the strongest free match for the quiz and poll side of SimpleForm, with knowledge quizzes, no-wrong-answer personality quizzes, and real-time polls all included at no cost. What Forminator does not include natively is any messaging layer: getting a Forminator submission into WhatsApp, Telegram, or Slack means adding Uncanny Automator or AutomatorWP as a separate plugin. SimpleForm’s quiz builder and poll creator match Forminator’s free feature set and add the messaging routing on top, inside the same plugin. OneClick Chat to Order solves a narrower problem well: a WooCommerce product page button that opens WhatsApp with the product details pre-filled. It requires WooCommerce, does not build general-purpose forms, quizzes, or polls, and has no Telegram, Slack, or Google Sheets option. SimpleForm covers that same “order via WhatsApp” use case through its own WhatsApp redirect, then extends it to every form, quiz, and poll on the site, with or without WooCommerce. So of the plugins that show up in a “wordpress form builder” or “form to WhatsApp” search, none combine a full form, quiz, and poll builder with native, free WhatsApp, Telegram, Slack, and Google Sheets routing the way SimpleForm does. That combination, not any single feature on its own, is what sets it apart from the rest of the list. External Services This plugin connects to external services only when the relevant integration is enabled or feature is used by the site administrator.
